Meals

To keep the cost of the training class reasonable, meals are not provided for this class. Attendees are provided with a break for lunch on their own. Drinks are typically provided by the host institution during the morning and afternoon breaks.

Cost

The cost of this class is $995.00 per person.

Payment Options

We are currently not able to accept credit card payments, so all payments must be made in the form of a manual check.

Upon submission of the registration form on the next page, the system will automatically send the attendee a confirmation of class enrollment. The email will also provide a W-9 form to enable the attendee to pay the invoice. (Please note: If you register for a 2025 class in 2024, a confirmation email will be sent immediately, but an invoice will not be sent to until early December).

The registration system allows the attendee to register themselves, or the system will allow a person to complete the registration on the attendee’s behalf. The attendee (or the person registering on the attendee’s behalf) will have an option in the system to request that confirmation be emailed to someone other than the attendee, if desired. Payment instructions and an invoice will be sent to this person via email, as requested.

The invoice must be paid within 6 weeks of the invoice date, so we ask that the invoice be processed by the institution immediately upon receipt.

If you do not receive a confirmation email within 1 business day, you want to consolidate invoices, or you need additional paperwork to process payment, please send an email to The DSA Team for assistance. Please be sure to check your “junk mail” or “spam” folders prior to contacting us if you are missing the confirmation email.

Requests for Reasonable Accomodations

Accommodations must be requested no less than 60 days prior to the start of the event.  DSA’s complete policy on accommodations and service animals can be found online at: https://www.dstaffordandassociates.com/nondiscrimination-and-accomodations/.
